Using Forms and Response-O-Matic

Forms can be a valuable and important tool if used properly. The only problems is that unless you have access to your server, have the appropriate cgi, etc., so the technical aspect has made it difficult for normal users to use them.

Now all of that has been solved through the miracle of e-mail and a web page called Response-O-Matic. This page sets up the form for you, it even writes the html for you. All you do is fill out one of their forms and a template is designed to send the information straight to your e-mail address.

There are several uses for forms, they include

  • providing a space for comments.
  • collecting information for a database. 

Here is an example of a Response-O-Matic form. You can have the results sent to you as a list or in a tab delineated format for easy insertion into a database.

There are several steps to creating and tailoring your form.

  1. Go to Response-O-Matic and set up a template (make sure to follow their directions explicitly!) Please note: The colors you are selecting are for page that comes up after you have submitted the form.
  2. Insert the html created by R-O-M into Home Page
  3. Change the form template to suit your needs
  4. Make your form database ready
